B) Monitoring Window & PRV Date (as stated)

Studio 3: "Up to 2 ft of wall removed on wall b and wall c of hallway side; walls and ceiling removed. Studs and beams HEPA and detergent cleaned. The area is free and clean of VMG and odors."

Upper Level Studio 3: "Ceiling tiles were removed. The area is free and clean of VMG or odors."

Studio 2: "The carpeting, ceiling, and walls were removed. The area is free and clean of VMG or odors."

Studio 1: "The walls, ceiling, and floor were removed as per work scope. The area is free and clean of VMG or odors."

Bathroom: "Shower wall removed; gypsum wall 3 ft from floor removed; water heater, sink, bathtub, tiles, and raised floor were removed. The area is free and clean of VMG or odors."

Hallway / washer‑dryer area: "Washer and dryer were removed; ceiling and 2 ft of wall from floor were removed (brick exposed). The area is free and clean of VMG or odors."

D) Measured Conditions & Air Sampling (report text)

  • Measured findings (day of PRV): "Building materials measured dry… Infrared inspection… measured dry with a temperature differential of 7 degrees (82.0°F–89.6°F). Relative humidity measured above the recommended range of 30–60% during the inspection."

Indoor: Temp 72.2–78.9°F; RH 72.9–73.0%.

Outdoor: Temp 75.0°F; RH 64%; "Partly Cloudy."

  • Air samples: 7 indoor + 1 outdoor control; table lists locations (Studios 3/2/1, Bathroom, Hallway, Kitchen Loft, Studio 3 Loft). The "Sample Results Interpretation" states indoor spores did not exceed the outdoor control in sampled locations.

E) Appendix B — Work Scope Plan (Created Dec 8, 2020) — as embedded in PRV

The PRV includes Appendix B: "Work Scope Plan; Created December 08, 2020". The scope (captioned "Mold Abatement Scope of Work — 97 Green St. Apt #G21") lists ten numbered tasks including, among others:

  1. Gut demolish rooms 1, 2, and 3"walls and flooring to the concrete slab," remove ceilings, ceiling tiles, and any insulation above; clean visible staining/mold on the wood deck; clean wood framing.
  2. Remove the bathroom raised floor under the tub and hot water heater; remove tub and hot water heater to access; clean and save fixtures.
  3. Remove the bathroom sheetrock wall shared with the kitchen; inspect cavity and back of cabinets.
  4. Move and save washer & dryer; remove the sheetrock wall behind them.

5)–7) Room‑specific wall removals (to cavity), hallway ceiling removal, remove insulation / clean loft and wood deck.

  1. Cut four 2×2 ft probes along the living‑room party wall to inspect cavity.
  2. Cut probes into the living‑room ceiling to the wood deck to inspect.
  3. Service and clean the air‑handling unit at completion.
